The Hospice Social Worker is a member if the interdisciplinary team whose primary function is to provide diagnostic assessments of the psychosocial and economic needs of the patient/family unit, define social service goals for alleviating these needs and help provide both the counseling and casework needed to meet these goals.


 


Please note: This is a per diem/as needed position that requires travel throughout our Macon service area.


 


Primary responsibilities include (as needed/requested):



  • Visiting patients to assess care and provide pain/symptom control

  • Managing a case load of hospice patients

  • Attending weekly interdisciplinary team meetings (as required)

  • Participating in care conferences and working in interdisciplinary teams

  • Supporting the patients' families with funeral arrangements, psychosocial needs, community resources, etc.

  • Maintaining hospice patients' charts and other documentation

Requirements

Required Qualifications:




  • Master's Degree-level clinician in Social Work


  • LMSW and/or LCSW is required


  • Strong written and verbal communication skills


  • Strong organizational skills


  • Strong computer proficiency must be able to type and navigate around a Windows environment


  • Must have reliable transportation including valid driver's license and automobile insurance

 


Preferred Qualifications:




  • Hospice experience


  • Experience working with community resources 


  • Home health experience


  • Long term care experience
