The SMArt Team Telemedicine Nurse Practitioner or Physician Assistant delivers primary care services to adult and pediatric patient via the telephone and sees patients in the NowClinic, as needed.  The SMAteam  NP - PA utilizes his/her triage, diagnostic experience and expertise to assess and treat patients presenting with primary care needs.  In addition to addressing the presenting problem, the SMArt Team provider will collaborate with the call center MAs and other extenders in the Telemedicine department.  NP - PA utilizes the telephone visit to engage patients in addressing wellness and gaps in preventive care, and provides an excellent and satisfying customer experience. 


 


Primary Responsibilities:



  • Provides primary care services to adult and pediatric patients via the Optum NowClinic virtual visit technology platform

  • Using experience and expertise, engages, assesses, triages and provides/prescribes treatment to patients presenting for care

  • Using sound medical judgment triages and directs patients to alternative levels of care for presenting problem and/or follow-up

  • Utilizes NowClinic technology to engage patients in addressing wellness and gaps in preventive care in accordance with evidence based practice and standards of care

  • Utilizes technology to thoroughly document visits and treatments in a timely and comprehensive manner in accordance with Optum NowClinic, medical group and regulatory requirements

  • Communicates effectively with patients, caregivers, colleagues, and support staff

  • Demonstrates and reinforces value of the SMArt Team service during each visit

  • Maintains service level standards for response, visit time, and patient satisfaction

  • Demonstrates and fosters a climate of collaboration and professionalism


 

Requirements

Required Qualifications:



  • National Nurse Practitioner certification in Adult, Family or Gerontology or

  • Physician Assistant certification

  • Licensure in the State where this position is based, or ability to obtain

  • 2+ years of hands-on clinical experience.

  • Intermediate level of proficiency in PC-based word processing including basic Microsoft Word and Outlook skills

  • Flexible to work occasional weekends, holidays, and evening shifts

  • Provide occasional coverage in Urgent Care/ Convenient Care Clinic

  • Ability to work independently from home based office 

Preferred Qualifications:



  • Strong acute care experience in an urgent care, retail clinic or primary care clinic

  • Knowledge of mental health issues

  • A background in managed care

  • Familiarity with ICD-9 coding

  • Tech Savvy
