Summary: Pathway to Choices San Rafael & Pinole is center based Behavioral Modification Program assisting individuals with a range of intellectual disabilities and emotional challenges in a 3:1 and 2:1 staffing setting. In order to help each program participant achieve their life goals Instructors will promote choices and engage each individual in meaningful activities in addition to teaching Independent Living skills. Instructors will provide Crisis Intervention, assistance with personal care, supervision within the facility, Community Integration and advocacy for all individuals. Essential Duties & Responsibilities: * Implement ISP goals and activity schedules for individuals served; * Implement Behavior Intervention Plans for each individual * Document Individuals progress toward goals and objectives, and other relevant information daily, weekly, monthly and annually; * Actively participate in meetings including but not limited to ISP meetings, Behavior Management Plan Sessions, Advocacy Meetings, Staff-Meetings, In-Service Training Sessions, etc. ; * Maintain and monitor professional relationships with consumers, families, Agency Staff, Regional Centers, Community Care Licensing, community members and employers; * Implement community based paid and volunteer opportunities for consumers; * Ensure consumer rights are protected * Assist in Personal Care of Individuals including but not limited to feeding, toileting needs, postural supports * Other duties and responsibilities as assigned. Qualifications: Six months of experience working with individuals with maladaptive behaviors and intellectual disabilities, Autism, Neurological Disabilities, Crisis Intervention, counseling, mental or behavioral health background or related or human services experiences preferred OR- Within six months of employment in the center Instructors will complete an additional 12 hours of continual education on the following topics: * Health and Safety * Consumer Right's and Advocacy * Communication and Engagement * Pro Act Training and Applied Behavioral Techniques and Plans for Individuals served * Individual Service Plans and ISP Goal implementation and IDT Meetings * Conducting meaningful activities * Developmental Disabilities * Conducting meaningful activities Instructors (regardless of experience) must have: 1. Within the first 40 hours of providing consumer services in the facility staff shall complete an on-site orientation which addresses the following: a) The program design b) Consumer's ISP c) Consumer's rights regulation d) Health and emergency procedures, including fire safety e) Identify and report all SIRs as mandated by Title 17, California Code of Regulations, Section 54327(a); and f) Identification and reporting of consumer abuse g) On the job training as necessary to implement consumer's IPP's h) Receive continuing education in one or more of the following: 1. Consumers' services as described in the program design 2. Promotion of consumers' rights, health, safety and social and physical integration; and 3. The ID Team process, development and implementation of IPP's. Other Qualifications: * High School Diploma or GED; * Must be at least 18 years of age; * Excellent verbal and written communications skills; * Excellent organizational, interpersonal, and decision making skills; * Compassion and empathy in dealing with people with a variety of physical, mental, emotional and social challenges; * Interest and willingness to work with consumers to meet their needs and choices; * Ability and willingness to be flexible; * Reliable Transportation; * Valid driver's license in good standing; * Car registration and vehicle insurance if providing transportation for individuals receiving services; * Successful clearance of local and/or state background checks; * Physical and TB Test Clearances
