- Recrui,t assess and enroll qualified recepients to the program.
-Assist clients with various services.
-Maintain a case load of 7 to 15+ clients.
- Implement intial and ongoing care plans. - Collaborate with internal and external providers to maintain a repore to provide exceptional seizes based on your client needs.
-Maintain regular client contact via homevists, phone calls, and various appointments.
Qualifications:
-Associate or Bachelors degree in Human Service, Nursing,Social Work related feilds.
-Minimum 2-3years working with families
-Must have dependable vehicle.
-Proof of auto insurance.
