Primary
Responsibilities:
- Engage
members face to face to educate about their health - Review
available member service records and relevant documentation (e.g.,
utilization history, functional level, stratification information, current
plan of care) - Conduct
comprehensive non-clinical member assessment that includes
bio-psychosocial, functional, and behavioral health needs - Utilize
interviewing techniques and active listening to collect and retain member
information and incorporating responses as they are presented to complete
assessment - Identify
member service needs related to health concerns - Identify
urgent member situations and escalate to next level when necessary - Work
with clinical staff to engage the member in participating in the
assessment process and collaboratively developing the plan of care based
on the member's individual needs, preferences, and objectives. - Work
with members to identify barriers to accessing care and achieving person-centered
goals, as well as work to close gaps in care. - Identify
member support systems available and incorporate into plan of care - Review
plan benefits and identify appropriate programs and services based on
heath needs and benefits - Work
with clinical staff to integrate health care and service needs into a plan
/ recommendations for member care and service. Transfer the member to
clinical staff to manage clinical needs, as identified. - Work
collaboratively with the interdisciplinary care team to ensure an integrated
team approach - Collaborate
with member to create solutions to overcome barriers to achieving
healthcare goals-Identify relevant community resources available based on
member needs - Refer
members to appropriate programs and services - Facilitate
member choice of preferred provider - Advocate
for individuals and communities within the health and social service
systems; - Role
assists in coordinating health services and does not write care plans - Help
members connect with transportation resources and give appointment
reminders in special circumstances. Transporting members is strictly
prohibited - Follow-up
with members via phone calls, home visits and visits to other settings
where members can be found
Requirements
Required
Qualifications:
- High
School Diploma/GED. - Knowledge
of Medical Terminology. - Ability
to create, copy, edit, send & save utilizing Word, Excel & Outlook. - 1+
year Medicaid/Medicare experience OR 1+ year as a Certified Community
Health Worker OR 1+ year experience working in the community/community
programs OR 1+ year working as a Case Manager. - 1+ year Telephonic Customer Service.
- Must
have reliable transportation, valid state drives license and required
insurance.
Preferred
Qualifications:
- Completion
of Community Health Worker Training program. - Associate's Degree.
- Previous
employment in a Medical Office, Hospital or Clinical or other provider
environment. - Bilingual.
