DESCRIPTION

This is a professional nursing position responsible for family centered care coordination management to children with chronic medical conditions. Included responsibility for this position is coordination of CMS specialty clinics.

EXAMPLES OF WORK

Provides care coordination to children with chronic handicapping health problems with attention to physical, developmental and psychosocial rehabilitation needs.

Provides accurate documentation of care coordination activities in client’s electronic record according to CMS Performance Standards. Completes yearly Redetermination Assessments, Health Teaching, and Transition Education as required.

Interviews clients and/or families applying for CMS services. Explains policies and procedures, while assessing family dynamics and completing forms necessary for sponsorship. Completes eligibility screening, initial assessments and care plans and education as per Children’s Medical Services Performance Standards.

Serves as a community resource person in the field of pediatrics. Coordinates patient services with other state and local health agencies to avoid duplication of services and to provide information to clients and other health care professionals. Participates in the Medical Home Initiative.

Performs clinic functions as assigned, to include charge responsibility, assuring case records are complete and accessible in the clinic, performing nursing histories and assessments, and assisting the physician with examinations and procedures. Conducts pre and post interviews with all clients and families during which the nurse reinforces MD orders, provides teaching and emotional support and documents outcomes.

Attends staff meetings, community meetings, and continuing education programs for professional development and licensure.

Responsible for participating in and responding to Emergency Operations and Disaster Recovery efforts as events arise within assigned office, region or state.

EXAMPLES OF KNOWLEDGE, SKILLS AND ABILITIES

Community Health and/or Pediatric Nursing experience needed. Ability to work with families and children with special health care needs. Clinic attendance and care coordination activities. Some traveling, mostly within Miami-Dade County.

LICENSURE, CERTIFICATION OR REGISTRATION REQUIREMENTS

This occupation requires licensure as a Registered Professional Nurse in accordance with Chapter 464, Florida Statutes or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27 or Certification as a Physician's Assistant in accordance with Chapter 458, Florida Statutes.
