The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, cerebral palsy, Prader-Willi syndrome, and intellectual disabilities. There is no charge or co-payment for services provided through the Agency.

APD customers may be served in their community by funding provided through several waiver programs. Other customers are provided assistance without being enrolled in a waiver program. Some APD customers are served at two Developmental Disability Centers. The Agency helps more than 50,000 Floridians with developmental disabilities each year.

APD provides a variety of medically necessary services to more than 31,000 customers who are currently enrolled in Florida’s iBudget Waiver. Those services may include occupational and physical therapy, private nursing, mental health counseling, personal supports, transportation, behavior therapy, residential services, adult dental services, and respite care.

Some people with developmental disabilities prefer to receive services and supports living in a home with other people with similar disabilities. The State of Florida has two centers: Sunland Center in Marianna and Tacachale Center in Gainesville. About 800 people currently live in these facilities. In addition, the Agency serves people found incompetent to go to trial due to their developmental disabilities. These individuals are served at APD centers and at a facility located in Chattahoochee. About 200 people live in these locations.

The State of Florida participates in the Bencor National Plan for OPS employees. The BENCOR National Plan includes two tax-qualified, defined-contribution retirement investment plans. Approved by the Internal Revenue Service (IRS) and offered by BENCOR, Inc. (BENCOR), the Plan is divided into two parts: the FICA Alternative Plan and the Special Pay Plan.

DUTIES:

This is a professional position responsible for medical case management and consultation throughout the Region. The incumbent works in an interdisciplinary and multidisciplinary fashion within the construct of the regional structure. Primary duties include, but are not limited to, clinical oversight of medical case reviews, medication administration training and monitoring, ICF/DD admissions, continued stay review, eligibility review, and other health related quality controls. Works collaboratively with state office medical/nursing team.

Serves as medical case manager to train and monitor Medication Administration Rule FAC 65G-7. Services include but not limited to coordination of approved RN trainers, monitoring compliance in licensed group/foster homes, technical assistance and training for support coordinators, supported living coaches, all providers and program office staff.

Coordinates responsibilities of OPS-Medical Case Management staff to complete medication administration monitoring, assignments, reports and follow-up, Level II PASRR, individual health reviews and eligibility.

Serves as coordinator for individual’s health related supports and services review, follow-up on reason for illness, injury or accident per case selection methodology or request.

Performs all ICF/DD facility admissions to determine level of care, all 180 and 360-day continued stay reviews in all District 7 ICF/DDs, maintain ICF/DD database, represents Agency for Persons with Disabilities at ICF/DD licensure surveys, and coordinates all activities necessary on the Cramer Suit.

Coordination of Level II PASRR Evaluation and Determination.

Provides consultation on eligibility reviews upon request.

Completes all reports of death per requirement, and mortality reports upon request.

Represents Agency for Persons with Disabilities in Hearings, meetings and client staffing where Medicaid issues are involved.
