Optum CarePlus (Evercare) is expanding and has an opportunity for a full-time Nurse Practitioner in the Albuquerque area for coverage at three to four long-term care facilities.

As an OptumHealth Nurse Practitioner (NP) you will provide primary care to your caseload of patients in long term care settings. Primary Responsibilities:
- Providing preventative and primary care for patients in three to five assigned long term care facilities covering 80-95 patients
- Working with primary care physicians to provide the best care possible
- Collaborating with the nursing staff and the patients' families
- An opportunity to learn palliative care is offered at Evercare
- Working with the facility's utilization review process
- Strong attention to customer service is essential
- Positions in this job function require specific education/certification/licensure and are considered an extension to Physicians and provide direct patient care in a clinic or in-home setting
Requirements
Required Qualifications:
- Current, unrestricted NP license in the state of assignment or the ability to obtain
- DEA License or the ability to obtain
- Nurse Practitioner national certification as ANP, FNP, or GNP required or the ability to obtain (Acute Care considered)
- A clinical background in adult, family or geriatric specialties preferred
- Past experience working in a nursing home or with seniors in other settings preferred
- Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook
- Hands-on post graduate Nurse Practitioner experience preferred
