Local travel to Emporia, Cottonwood Falls, Lebo, and Strong Kansas


Primary Responsibilities:


  • Engage members telephonically and onsite to coordinate services, community resources, and treatment needs

  • Deliver a holistic approach to coordinated care based on the enrollee needs

  • Identify early risk factors and conduct ongoing assessments and document in an electronic file

  • Create, review and revise Plan of Care plans and focusing on disease management

  • Function as an advocate for members and decrease unnecessary hospitalizations

  • Collaborate with the member's PCP to deliver and coordinate necessary services

  • Build relationships with members and their families; assist them with proper health choices

  • Ensure cases are documented in a timely manner

Requirements

Required Qualifications:


  • Current, unrestricted SW or RN license in the State of KS

  • 1+ years of Case Management experience

  • Valid driver's license and reliable transportation

 

Preferred Qualifications:


  • 1+ Home Health, in-home service experience

  • Medicare/Medicaid experience

  • A background working in geriatric special needs

  • A background in behavioral health

  • Experience with electronic charting

  • Ability to multi-task

  • Certified Case Manager (CCM)

  • A Background in managed care

  • Computer proficiency with MS Word, Excel, Outlook and navigating the Internet
