Key responsibilities include:
* Implementing and coordinating the delivery of care in collaboration with physician and Interdisciplinary Team, patient and family.
* Completing an initial and comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 
* Provides healthcare instructions to the patient as appropriate per assessment and plan.
*Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency adn optional comfort care.
* The Case Manager assumes responsibility to coordinate patient care for assigned caseload by the Clinical Supervisor.

We seek the following principal qualifications:
* RN state licensure in good standing
* 1 year previous Med-Surg, ICU, ER or hospice experience preferred but will consider the right candidate.
* Enjoy working with the terminally ill patient and family
