This includes client intake, quality assurance and supervisory visits, care conferences, as well as care consultations with potential clients and family members. The RN Client Care Coordinator also supervises our home health aide staff. Qualified candidates will be a team player that possesses a positive attitude with exceptional communication and organization skills. Care coordination and home care experience preferred.
