Employees in this function require a current, unrestricted LPN license in FL. These roles identify, coordinate, and provide appropriate levels of care under the direct supervision of an RN or MD.  This function is responsible for Clinical Operations and Medical Management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating); This includes Case Management, Coordination of Care, and Medical Management Consulting. This function may also be responsible for providing Health Education, Coaching, and Treatment Decision support for Members. 



This position requires ACHA Level II Background
checks (fingerprinting) by the State of Florida for all Clinicians that have
face-to-face contact with members and will require renewal every five years.




Primary Responsibilities
  • Initiates face to face visit at home post discharge from an institution and assess that member needs are met. 
  • Visit the member onsite to review the physician orders, assess the patient's clinical needs and gather clinical information.
  • Collaborates with colleagues in the interdisciplinary team to insure quality driven, best practice and effective transfers in the movement of patients across the care continuum.
  • Coordinate arrangements to meet the needs of the member prior to discharge from institution an (i.e. HHC, My Home Doctor Services, MSW evaluation, PCP appointment.) 
  • Completes medication reconciliation process to avoid medication errors such as omissions, duplications, dosing errors, or drug-drug interactions.
  • Facilitates timely access to care, appropriate utilization network and community resources and optimal coordination of medical treatments across the full continuum of care in order to improve clinical outcomes for plan members.
  • Provides support to the member in identifying concerns and problems and builds relationships with practitioner.
  • Participates in and/or facilitates transitional care meetings and activities. 
  • Attends and Participates in interdisciplinary team meetings as directed. 
  • Identifies high risk/ high cost patients for possible case management interventions.
  • Maximizes positive financial outcomes for designated patient populations by intervening at key points in the disease process with home visits, post discharge follow up, educational materials, phone calls to promote health and prevent readmissions. Conduct Home visits, post discharge follow up, and phone calls to promote health compliance.

Requirements

Required Qualifications:
  • High School Diploma/GED.
  • Active Florida LPN license.
  • Bilingual in English and Spanish.
  • 1+ year of experience using a computer and Microsoft Office.
  • Must be willing to travel 100% of the time within assigned territory.
  • Must possess organizational skills.
  • Must be able to identify problems and implement solutions.
Preferred Qualifications:
  • Case Management experience.
