Description SUMMARY

Coordinates and administers all participant benefits, services, admissions and marketing/communication efforts for all relevant applicable programs.

ESSENTIAL JOB FUNCTIONS

To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.

  • Establishes and maintains relationships with potential referral sources and provides information on Mentor Network programs.

· Facilitates determination of referral viability, clinically and financially. Checks benefits for health insurance using benefits checklist, contacts precertification and establishes contact with a case manager.

· Completes all funding verification protocols. Informs evaluator of benefits profile. Identifies all types of funding possibilities including primary, secondary, and ancillary. Upon approval, submits information to funding source by overnight, letter or fax.

· Works with funder, program director, evaluator, and family as necessary to assure that admission is scheduled and everything is in place for admission.

· Completes and processes intake form, notifies evaluator and sends documents for database input. Distributes records and other documents to appropriate staff for clinical and medical approvals; tracks and documents status of clinical and medical approvals.

· Tracks and records admissions, discharges, new referrals and closed referrals on pathway. Determines reason for closed referrals and records in file and in database.

· Manages “800” call and web-site referral system. Takes calls and facilitates an immediate response by each region to caller’s requests. Provides research and resources to outside and internal callers regarding programs and product lines.

  • Provides administrative assistance to marketing; orders and maintains inventory for brochures, marketing collaterals/premiums, business cards, letterhead, envelopes, etc. as required.

· May facilitate design and distribution of newsletters, fliers, advertising and brochures.

· Performs other related duties and activities as required.

Qualifications Minimum Knowledge and Skills required by the Job

The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.

Education and Experience:

High School Diploma or equivalent and three years experience in rehabilitation service provision. Associate’s or Bachelor’s degree preferred.
