Supervisor: Head QIDP/ Administrator
Classification: Hourly 40 hours a week (Flexible hours based on Individual's needs)
Basic Function:

To provide quality support services to individuals that enhance physical, emotional, intellectual, vocational, communication and social skills according to individual's needs, abilities, and choices. The QIDP is responsible for planning, coordinating and implementing all educational, vocational and recreational programs for the individuals at UCM. The QIDP is the chairman of the interdisciplinary Team meetings (ISP Reviews) and is responsible for this group of Professional Team Members. The QIDP is to exercise responsible professional judgment in planning/reporting to, and receiving general policy and administrative guidance.

Primary Responsibilities:

* To coordinate/ develop, plan, write, and implement success programs which will enable each individual to achieve maximum capabilities.
* To give proper input during the Interdisciplinary Team meetings (ISP reviews, Behavior Management Committee Meetings, Human Rights Committee Meetings) in the programing area.
* To work with the Interdisciplinary Team in developing an individual program for each individual.
* To assist in the training of the Direct Support Providers and other staff relative to training programs identified through the ISP process.
* QIDP is responsible for training Direct Support Staff on individuals programs though program observations. Allowing QIDP to gain information on what an individual's skill level is with a specific task as well as showing or allowing staff to show QIDP what works best with each individual.
* Observe, and train Direct Providers as directed by the Director of Health Care Services.
* Demonstrates leadership skills and introduces new active treatment program materials and equipment to staff.
* Develops and maintains channels of communication between departments, outside agencies, administration, and volunteers.
* To assist in the training of family members and staff to build relationships and to carry out individuals rights.
* Receives and reviews reports from staff members, consultants, and the Interdisciplinary Team member's daily/ monthly.
* Keeps Program Director informed on all aspects of the DSP and individuals; including, but not limited to problems/ needs, makes recommendations and implements recommendations.
* Maintains all necessary files, makes reports as required and participates in the development of manuals, informational or program materials and annual reports.
* Maintains an active interest in professional organization and related activities to enhance the life of the individual's life.
* Perform other duties as deemed necessary by the direction of the Program Director to assist in the overall responsibilities of the department.

* Attends meetings and events appropriated to the promotion and enhancement of the individual and the corporation.
* Functions as a counselor, friend and advocate for the individuals.
* Will participate in Active Treatment process, follow behavior management policies and procedures, and will understand the concepts of the programmatic services.
* Work schedule requires regular flexibility to ensure effective supervision of individual programs.
* Reviews, monitors, and revises programming through completion of Quarterly and Annual QIDP reviews to provide the best possible services for each individual on the assigned caseload.
* Investigate MUI/ UI for individuals on caseload and cover individuals on others caseloads as needed during times that they are not available to ensure that they are being completed in a timely manner.
* Work with County boards to ensure that all information needed for MUI/UI's have been faxed/ scanned to the appropriate person to close investigations.
* Ensure all paperwork and reports related to MUI/UI's are completed in a timely manner and have been received by all who need them.
* Must follow schedule/ be flexible with others working on the OnCall Schedule to ensure sure that days are covered for reporting/ notifying appropriate individuals for MUI's, and go to the hospital with individuals during your scheduled OnCall days.
* Track and monitor programs monthly for necessary changes ensuring that programs are meeting frequency and making changes in a timely manner.
* Keep accurate and updated notes on orders, nursing, purchases, conversations with guardians etc.
* Responsible for filing all information into the habilitation charts and other books as necessary.
* Complete assessments of individuals abilities annually making adjustments as needed.
* Ensure proper consent is received for implementation of programs and individuals plans.
* Ensure all staff are inserviced on all changes to individuals plans.
* Participate in orientation training of new staff, explaining key points under QIDP section of training.
* Thoroughly review peers annual /quarterly reviews and consultant inputs to ensure consistent and accurate information is in the ISP.
* QIDP will contact guardians on a routine basis, conducting themselves in a professional manner. Guardians should be contacted by QIDP's for consent to changes in ISP as well as any other important information that affects an individual.
* QIDP is responsible for assisting in transportation of individuals to and from day services. This includes loading and unloading individuals, driving vehicles as needed, strapping down wheelchairs, assisting individuals with coats and gloves.
* QIDP will be needed to assist the aids on the floor with tasks such as feeding in the dining room, answering alarms, transferring an individual, or assisting an individual with a need.
* QIDP is responsible for making sure individuals on their caseload have personal items that are needed and are in good shape. Items should be free of holes and stains and an individual should have enough items for 7 days. QIDP should be responsible for buying individuals such items as socks, underwear, bras, shirts, pants, shoes, etc.
* QIDP is responsible for marking all items purchased with the individuals name so items are sure to be placed in the correct individuals rooms.
* QIDP will be given receipts for purchases made by the individuals during the month. QIDP is responsible to make sure that all receipts are signed by the individuals on their case load.
* QIDP will at times accompany individuals on their physician appointments. QIDP should conduct themselves in a professional manner as they represent UCM Residential Services and the individual. QIDP should make sure that they are familiar with the individual and why they are going to the appointment and any issues prior to the appointment, so they can best answer an questions the physician may have.
* QIDP is primarily responsible for the psychological appointments with Access Ohio for the individuals on their caseload. These are mainly teleconferences held on the computer with the Q and the individual and at times a nurse. QIDP must ensure that new orders are relayed to the nurse so they can watch for the orders to be faxed from Access if a nurse is not present for the teleconference.
* QIDP's should work together with the team to find roommates that work best together within the ICF. QIDP's should work together with other staff to move furniture should a room change need to be made. Ensuring that all wall decor and personal possessions are moved with the individual and that they all get hung back up and TV/radios hooked back up.
* Special Duties
- Keep accurate minutes from Human Rights Committee meetings.
- Ensure that MUI/UI logs are completed accurately and entirely. (i.e. all follow up documented, all follow up completed.

Qualifications:
* Must have a 4 year Bachelor's degree.
* Prefer at least six months to a years' experience but not required.
* Must possess strong leadership abilities and the willingness to work effectively with other staff.
* Must be able to pass all pre-employment screenings.

Physical Demands:

This position will require standing, sitting, walking, bending, stretching, lifting of moderate to heavy amounts, pushing and pulling for moderate periods of time. May involve a degree of risk to back injury if proper lifting procedures are not followed. Additional hazards may include injuries related to behavioral incidents during interactions with individuals.

Work Environment:

This position may be required to work flexible hours depending on the needs of individuals. Maybe required to work late hours or weekends due to on-call schedule. This schedule is rotated with other staff and staff are able to request to trade amongst themselves. The work environment for this position may include risk of exposure to certain communicable diseases if proper hygiene techniques and proper protection apparatus are not utilized. This position may require you to from time to time travel between locations.
