Clinical Case Manager (RN)

Our 521 bed hospital has provided state-of-the art medical care for residents of the Roanoke Valley and Southwest Virginia for more than 100 years. At LewisGale Medical Center, we specialize in select health care services, such as cardiac, cancer and maternity care, as well as business health and other ancillary services. This position will play a key part in continuing our success!

LewisGale Medical Center is currently looking for a Clinical Case Manager (RN) .

Responsibilities:

Applies the rules of Severity of Illness, Intensity of Service, and Discharge Screens/Indicator consistently in evaluating medical necessity.

Performs admission and concurrent review for all payers and maintains legible documentation as directed by the Utilization Management Plan.

Participates in huddles to facilitate patient throughput.

Educates patients and families to the patient's needs post discharge and informs them of the alternatives available to meet those needs.

Refers patients who require extended care post discharge to post-acute care facilities. Once an appropriate bed is located, arranges for the patient's transfer.

Collaborates with community agencies to provide services to assist with housing, financial, transportation, psychosocial, educational, home health, and home medical equipment needs of patients/families post discharge.

Provides interdisciplinary and collaborative care, treatment and services; initiates patient/family conference with the health care team to achieve positive outcomes as indicated.

Identifies, tracks, and records Alternative Care Days (ACDs). Intervenes to prevent or decrease ACDs as appropriate.

Documents case management assessments, treatment goals, interventions, outcomes, and discharge plan in the patient's medical record congruent with departmental policies.

Initiates denial and appeal process in accordance with policy.

Refers HINN letters to Director.

Maintains knowledge base for identification and reporting of potential patients with contractual adjustment needs and length of stay issues.

Provides crisis and supportive counseling to enhance the patient's and family's problem solving and coping skills.

Counsels patient/families regarding advance medical directives.

Takes a proactive role in discharge planning.

Provides ongoing education on care coordination for the other members of the health care team.

Maintains current knowledge of trends related to case management and care coordination issues through attendance and active participation in continuing education and professional organizations.

Other duties as assigned.

Qualifications:

Two years of recent clinical experience as RN.

Five years of recent clinical experience as RN; Commercial Utilization Management and/or Case Management experience preferred

Quality Management experience preferred

Case Management certification preferred.

Registered Nurse with current Virginia licensure.

BSN or enrollment in BSN program required within first two years of employment and completion of BSN within five years of employment
