This position is responsible for ensuring the
continuity and coordination of quality, cost-effective care while minimizing
fragmentation of the healthcare delivery system for ESRD and CKD patients. This
position provides coordination of care between the patient, family, physician,
provider and care teams, the community, Acute care facility and Clinical Operations
Manager. The ideal candidate facilitates continuum of patients' care utilizing
advanced nursing knowledge, experience and skills to ensure appropriate
utilization of resources and patient quality outcomes. Performs care management functions on-site or
telephonically as the need arises.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties and responsibilities
reflect the expectations of this position but are not intended to be all
inclusive. Identify and assist with needs of patients including but not limited
to medical, social, emotional, and financial concerns through medical record
review, consultation with patient, family, and healthcare team members
Obtain
appropriate referrals or appointments for members
Ensure
appropriate parties receive required information
Evaluate,
coordinate, and facilitate patient and/or family access to necessary and
appropriate health, human, and community services
Identify
and redirect use patterns to maximize member benefit and care Implement
innovative nursing interventions appropriate to short- and long-term needs
of patients; ensure care coordination to complement and support medical
treatment plans while integrating psychosocial, physiological, and
discharge planning needs
Coordinate
healthcare services to ESRD and CKD members across continuum of care;
provide follow up for member care; evaluate outcomes
Manage
member care plans to support effective and appropriate medical cost
management and reduce number of hospital re-admissions through proactive
discharge planning Uphold Patient Bill of Rights and right to
confidentiality at all times Know and comply with:
DaVita
policies and procedures in accordance with state and federal regulations
for patient care and services (CMS, HIPAA, OSHA, or other governing
agencies)
Standards
necessary to maintain organizational accreditation (such as NCQA)
State
or federal regulations of the Injury Prevention and Safety Training
Program Other duties and responsibilities as assigned
Ability
to adjust schedule to accommodate organizational and/or customer needs
with limited notice
Attend
staff meetings, phone conferences, and training as needed
Know,
understand, and follow teammate policies and procedures
Maintain
and meet all annual competencies and skills requirements Travel required:
up to 50% within assigned area

MINIMUM QUALIFICATIONS
Education, licenses, certifications, and
experience required to fulfill the essential duties, include computer skills as
required. A.D.N degree from accredited school of nursing required; B.S.N
preferred; three-year diploma from accredited diploma program may be
substituted for nursing degree Current RN license in the state of practice
required; continuing education credits maintained as required by state of
practice required Minimum of five (5) years' experience in clinical nursing
required
Minimum
of three (3) years' experience in nursing preferred
Demonstrated
knowledge and understanding of data and managing to clinical, financial,
and patient satisfaction outcomes
Demonstrated
experience and effectiveness in change agent role
Demonstrated
knowledge and understanding of CQI techniques
Previous
experience in healthcare performance coaching required
Certified
Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
Current
CPR certification required
Ability
to modify personal practice patterns to adapt to new / electronic
processes and increased productivity expectations as it pertains to
Capella implementation
Current
driver's license in state employed with positive driving record and able
to meet requirements of insurance coverage required
Basic
computer skills and proficiency in MS Word and Outlook required
Functional
proficiency with DaVita specific clinical software programs, including
Capella, required within 90 days of employment
Home
office with internet connectivity at minimum of 1MB upload and 1MB
