ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all-inclusive.

* Make outbound calls to DaVita patients to schedule a TMR.

o Contact patients that are recently discharged from a hospital or are new to dialysis.

o Follow-up with targeted patients on a regular basis to ensure TMR is completed.

* Provide feedback and documentation support as necessary to ensure department policies and procedures (SOPs), knowledge database, and processes are current

* Provide support to other departments as needed

Other duties and responsibilities as assigned including but not limited to:

* Work overtime with little or no notice as needed

* Attend and/or lead team meetings, phone conferences, and training as needed

* Know, understand, and follow teammate guidelines, employment policies, and department or company procedures

* Consistent, regular, punctual attendance as scheduled is an essential responsibility of this position

MINIMUM QUALIFICATIONS

Education, licenses, certifications, and experience required to fulfill the essential duties, include computer skills as required.

* High school diploma or equivalent required

* 4 years experience as a pharmacy technician required

* High level of communication with difficult to understand or hard to reach patients

* Superb organization; Ability to prioritize and accomplish a heavy volume of work in a finite amount of time.

* Pharmacy Technician Certification required; demonstrate and maintain good standing with State Board of Pharmacy

* Typing speed of 40 wpm required

* Intermediate computer skills and proficiency in MS Word, Excel, and Outlook required
