We are seeking a knowledgeable, compassionate Nurse to provide direct care to hospice patients in their homes or skilled nursing facilities. As a PRN Hospice Nurse, you will work with other healthcare professionals to determine the best overall care for your terminal patients (prognosis of 6 months or less). Additionally, you would provide emotional support to the patients' families. This position offers potential for growth, a friendly staff, and an opportunity to interact with patients and their families on a more personal level.  

Please note: This position is responsible for working Per Diem/PRN; as needed. Our PRN nurses will provide coverage for vacations, leaves of absence and sick leave and will be available days, evenings, nights and weekends as needed. This is a field based position and the PRN RN will need to be available travel throughout our hospice service area as needed.  

Primary Responsibilities:  

• Conducting routine visits at home or nursing facilities


• Conducting admissions


• Coordinating, assessing, and implementing patients' care


• Providing accurate and timely documentation of patient services to reflect plans of care


• Overseeing and participating in day to day clinical care of terminal patients and their families


• Interacting with other group members such as CNA's, LPN's, social workers and dieticians


• Attending weekly interdisciplinary team meetings (as requested)


• Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials

Required Qualifications: 

• RN licensure (current, unrestricted) in the state location of position


• 3 years of clinical experience is required


• Hospice experience


• Reliable transportation including valid driver's license and active auto insurance

Preferred Qualifications: 

• Excellent communication skills


• Experience in long-term care is a plus


• Computer proficiency is a must


• Experience in long term care
