About this job

The Home Health and Hospice Aide (HHA) performs under the direction and supervision of the RN Case Manager, through delivering patient care activities and related services necessary in caring for the personal needs, safety, and comfort of patients as assigned. In this role the HHA may/will assist with patient care activities including but not limited to: oral hygiene, hair care, nail care, patient positioning and ambulation, bathing, bowel and bladder functions and care during the death and dying process. The HHA will work as a member of the patient care team, reporting changes in patient condition efficiently with accuracy to the RN case manager. The HHA is also responsible for completing and submitting all visit/patient related documentation. The Home Health and Hospice Aide will assist patients in accordance with the patient's plan of care, in compliance with all state and federal regulations. The HHA will report to the Hospice Clinical Supervisor.

Education:

Home Health Aide training verification or CNA certification in the state of practice preferred.

Experience:

Prefer a minimum of three (3) months' work experience as a Nursing Aide, preferably in home healthcare or hospice environment.

Licenses/Certificates:

Home Health Aide training verification or CNA certification in the state of practice preferred. Current CPR card. Current Automobile insurance. Current driver's license.
