POSITION SUMMARY: The Companion Aide is an in-situation champion of person-centered dementia care; a supportive presence for the resident. This does not preclude being a supportive presence for co-workers who are also involved with that resident. For example, it is reasonable to expect that a Companion Aide might accompany his/her resident(s) to a recreation activity even though the recreation therapist might usually do this. RESPONSIBILITIES/ACCOUNTABILITIES: 1. Get to know who the resident is and communicate the "who" to the care team; 2. Interact with family and resident to get information on the resident's life and preferred daily routine; 3. Be present and listen to the resident; 4. Help identify unmet needs and help to meet them; 5. Attend the resident's care planning meetings and use the "who" and observations of unmet needs to help drive care; 6. Create and monitor use of the Music and Memory intervention; 7. Accompany the resident on walks (indoors and out of doors); 8. Identify preferences and engage the resident in individualized and small group activities; 9. Support participation in resident's preferred activities that enhance quality of life; 10. this could include, but need not be limited to, creative, recreational, spiritual and social activities; 11. Encourage residents to eat or take nourishments; and 12. Assist the staff development coordinator or OASIS trainer with "just-in-time" trainings on person-centered dementia care as able. 13. Performs other duties as requested. ACN1

Qualifications:

SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS: 1. Licensed Nurse Aide with training in dementia care with at least two years of experience working in a nursing home setting. 2. The staff person must attest that the role is chosen, not assigned. In other words, to qualify, the employee must want to do this. 3. Person-centered dementia care training in the first year: 4. Satisfactory completion of the facility's basic dementia care program (as provided by Alzheimer's Association). This must be evidenced by a passing score on an examination or a certificate of completion. 5. Satisfactory completion of: 6. Complete basic dementia care training (as provided by Alzheimer's Association, others), 7. Complete basic TBI training (on-line Michigan course), and/or Complete Oasis, Hand and Hand or other approved CMS trainings and pass core competencies at end of each module. 8. Person-centered dementia care training in year two--each Companion Aide must take an annual refresher on person-centered dementia care using an approved curriculum by DAIL. 9. Be in good standing with the facility as evidenced by at least a satisfactory performance evaluation during the previous rating period. 10. This position requires that the employee is able to read, write, speak and understand the spoken English language to ensure the safety and wellbeing of our patients and visitors at the work site when responding to their medical and physical needs. 11. Must provide verification of TST (tuberculin skin test) as required by state law and in accordance with Company policy. TSTs will be administered at the work site if required.
