About this job

1. Adheres to all Brookwood Home Health policies and procedures and maintains standards of practice and documentation that are consistent with State and Medicare regulations and accreditation standards.

2. Upon acceptance of a patient, follows through with all visits ordered throughout patient episode. Reports to the office after the last visit, completes paperwork, and gives Director of Nursing updates on patients.

3. Evaluates patients, families, home environment for appropriateness of admission and verifies the patient's eligibility for the Medicare home care benefit, including homebound status, at the time of initial assessment visit and comprehensive assessment.

4. Participates in case conference/coordination of care communication.

5. Performs all duties of case management for assigned case load.

Current RN license in the state of AL; Must have CPR certification. Minimum of 1 year RN experience required. Knowledge of Medicare coverage criteria for therapy services to Home Care Patients. Ability to audit charts for documentation, QI and UR purposes. Current driver's license, reliable transportation, and auto liability insurance.
