Regional RN Care Navigator
Trinity Senior Living Communities (TSLC,) is seeking a Regional RN Care Navigator for our long-term care communities located in the SouthEast Michigan Region of the United States. TSLC operates 39 communities in nine states with annual revenues of 192M. TSLC provides a competitive compensation and benefit package that complements this opportunity for personal growth and professional advancement. This position reports to the Director of Care Navigation Services and is field based requiring 90-100% travel.
Under the supervision of the Director of Care Navigation Services, the Regional Care Navigator is responsible for the day-to-day management of Care Coordination in conjunction with TSLC Community-based care navigators/care managers who will develop and manage care plans for patients within a bundled, ACO, and/or managed care environment within an assigned region.
General responsibilities will include:
Responsible for uniformity in the regional implementation of clinical guidelines to effectively coordinate care throughout the healthcare continuum.
Leads any potential certification and accreditation projects related to Care Coordination and Case Management
Ongoing implementation of Customer Advocate service standards including quality auditing, training and development program.
Monitor and manage community-based care navigator/care manager performance within assigned region through technology and reporting, and develop action plans to maintain the highest level of service in the markets that are being served.
Develop and implement quality program including Medicare star rating standards that are appropriate for the scope of work, but aligned with quality incentives within assigned region
P rovide administrative planning and participate in strategic planning for the department, including both areas of intake and care coordination and will be the regional liaison for all external agencies that provide care to participants
Substantial travel including some overnight travel required
Candidates will have:
Previous experience in healthcare required, long-term care and/or home health industry preferred.
Strong understanding and application of the quality process and tools
Candidates should have excellent communication and presentation skills, with the ability to communicate with multiple levels in the organization.
Candidates should also possess the ability to work as part of a team, coach and train staff, as well as strong analytical and problem-solving skills.
Knowledge of general management, strategic planning and marketing principles as they relate to the operations of health care facilities.
Must possess the ability to seek out and develop new methods and principles and be willing to incorporate them into existing practices.
Must demonstrate analytical ability and possess a comprehensive knowledge of basic data analysis and communication tools including, Excel, Access and Word.
Must be comfortable operating in a collaborative, shared leadership environment.
Education Requirements:
Bachelor's degree in Nursing, or related field from an accredited college/university and a minimum of three years of experience in Care transitions, Managed Care, Disease Management, Utilization Management/Review, or related health care call center scope of work; or equivalent combination of education and experience. Master's degree in Nursing, Health Care Administration, Public Health or Social Work preferred
Health Coach Certification or active pursuit of certification preferred through the National Society of Health Coaches. Case Manager Certification or active pursuit of certification preferred
