BASIC FUNCTION: Assumes the responsibility for managing of total patient care

in the home by the provision of comprehensive skilled nursing care geared toward

continuity of care, care directed toward established goals and outcomes,

documentation reflecting care given, and discharge planning.

The staff member must be able to demonstrate the knowledge and skills to provide

care appropriate to the age of the patients served on his/her assigned unit or within

his/her department including knowledge of the principles of growth and

development over the life span. The employee must possess the ability to assess

data reflective of the patient's status and interpret the information needed to identify

each patient's requirements relative to his/her age-specific needs. It is also

necessary that the employee be able to provide the care needed as described in

the unit's/area's/department's policies and procedures. The staff member must be

flexible and willing to work in what capacity needed as deemed by the clinical

supervisor and/or director.

The clinician will be used at times to cover the office as needed and will be included

in taking call rotation, holiday rotation, as well as weekend backup.

III. SIGNIFICANT RESPONSIBILITIES AND ACCOUNTABILITIES:

1. Provides quality skilled nursing care, assessment, and treatments to

those patients assigned. Responsible for coordinating all aspects of

care to those patients assigned and documenting such.

2. Responsible for completing and submitting documentation in a timely

manner as well as for maintenance of clinical record.

3. Responsible for follow-up of documents mailed to the physician for

signature.

4. Rotates weekend schedule.

5. Admits patients to Circle Home Care as assigned.

6. Initiates the patient's plan of care and necessary revisions in

accordance with nursing needs specified in the approved plan of

treatment.

7. Conducts in-home supervisory visits when the home health aide or

nursing support employee being supervised is present and is

delivering services to the patient. Also supervises and teaches other

nursing personnel as assigned.

8. Reports changes in the patient's medical or mental condition to the

attending physician and to agency employees and contractors involved

in the patient's care.

9. Teaches or supervises the teaching of household members who are

responsible for assisting the patient with his nursing or personal care

needs.

IV. JOB SCOPE: Provides quality skilled nursing care to meet the physical, emotional and psychosocial needs of the client based on nursing process. Communicates with doctor and interdisciplinary team regarding patient status/progress. excess of 200 pounds four to six times a day. RN may need to carry equipment and supplies in excess of 50 pounds. Frequent exposure to temperature extremes,

depending on individual home environment as well as seasonal weather conditions, i.e. rain, snow, sleet, wind and high humidity. Worker is required to operate motor vehicle on a daily basis in carrying out patient assignments. There is frequent exposure to environmental elements, i.e. sun, potential exposure to chemicals in blood tubes, etc., exists. Potential exposure to blood and body fluids occurs with almost every nursing function. Contact with fumes, dust, etc. may occur in travel as well as inside patient homes. The RN is independent in the field and must be able to interact with families. Shift work occurs with p.m. visits and PRN calls. Patient homes often provide cramped working quarters. Stressful documentation requirements exist. Frequent visits in crime areas, as well as remote areas occur. Mental and physical stress encountered frequently in on-call situation.

Education: Graduate of an approved School of Nursing.

Experience (Type and Years/Months): Must have at least one year's

experience in nursing. Home Health experience preferred, as well as

med-surg/pediatrics.

Licenses, Certifications, and/or Registrations: Registered Nurse licensed

in the State of Virginia. Valid driver's license.

Other Minimum Qualifications: Must have own form of reliable

transportation.
