MENTOR Oregon supports adults and children with intellectual and developmental disabilities, acquired brain injury and other complex needs in a variety of community-based programs throughout the state. Through a range of individualized programs including Residential Services, Supported Living, Vocational Services and Foster Care for children with medical and behavioral support needs, MENTOR Oregon strives to help individuals of all abilities live life to the fullest in the communities they call home. MENTOR Oregon also operates an innovative Brokerage Services program, which coordinates self-directed support services to over one thousand Oregonians with developmental disabilities so they may live as independently as possible.

Description

MENTOR Oregon is looking for a full-time Personal Agent in Portland, OR. Mentor Oregon Brokerage provides service coordination for 720 adults who experience intellectual and developmental disabilities and who live in Multnomah, Clackamas, and Washington counties. Mentor Oregon Brokerage believes in self-determination. We are a team of employees who work well together to ensure services are delivered in the ways customers want them.

A Personal Agent coordinates services for up to 45 individuals living in their own homes. Personal Agents use a person-centered approach to assess the individual strengths, needs, and preferences of each customer, then develop individual support plans (ISPs) with the person to meet their needs and work toward their goals. Personal Agents coordinate services for customers by developing the customer’s natural support network and by connecting them to community resources and paid support providers. Personal Agents are responsible for regular monitoring of ISPs, addressing customers' changing needs (both immediate and long term), as well as ensuring customer choice and satisfaction with the services they receive. Personal Agents manage Medicaid funds and monitor ISP services and expenditures.


Qualifications

Requirements and Characteristics:


  • Must have an undergraduate degree in a human services field and at least one year of work experience in the intellectual/developmental disability field;

OR


5+ years of equivalent training and work experience related to developmental disabilities

AND

Knowledge of the public service system for developmental disability services in Oregon

  • Strong communication skills, including both written and oral forms;
  • Skills in meeting facilitation and conflict resolution;
  • Ability to work independently and efficiently with attention to detail, demonstrate strong time; management skills, organizational skills, strong math and budgeting skills, meet established deadlines, as well as have the ability to adapt to change;
  • Strength in comprehending, interpreting, and implementing a wide variety of established policies, procedures, and the Oregon Administration Rules;
  • Proficiency with Microsoft Office computer programs, and comfortable with use of a database;
  • Commitment to choice for individuals receiving services and focus on self-determination;
  • Prior experience in service coordination preferred

  • Must pass a criminal background check
  • Must have access to a working vehicle, and the ability to safely and efficiently meet customers in their homes and other community settings throughout Multnomah, Clackamas, and Washington Counties
