Physical Therapist / Therapy

Performs evaluations and develops effective patient treatment plans to restore, maintain or prevent decline of patient function, by planning and administering medically prescribed therapy treatments in accordance with federal, state and professional standards governing home health. Services performed primarily in individual patient homes.

Graduate from an accredited program for Physical TherapyCurrent license as a Physical Therapist in the state of practice as requiredAt least one of experience in a health care setting.Must be able to travel to patient homes within designation geographic area.Valid driver's license and proof of auto insuranceMust meet Clinical Competency requirementsMust have and maintain current CPR certificationMust be capable of maintaining regular attendance / Must be able to communicate in English, both verbally and in writing Possess problem-solving skills of the type and at a level necessary to accomplish the job Must possess computer skills of the types and at a level necessary to accomplish the job Must be able to maintain confidentiality regarding patient and company proprietary information Ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels
