Under the direction of the Home Health Manager/Clinical Coordinator, the Home Health Care Aide performs duties that provide for the activities of daily living. Performs related duties as assigned.

Provides competent personal care assistance to Home Health patients in a safe and effective manner. Performs only those skills for which competency has been determined and requests assistance as needed

Follows HHA assignment sheet as outlined by an RN.

Demonstrates the ability to prioritize assignment based on patient need and satisfaction.

Identifies observations regarding the patient's health status, environment and/or satisfaction with Home Health service and reports these observations to a team leader in a timely fashion.

Assists patient to achieve maximum self-reliance.

Performs all task demonstrating knowledge of patient safety and infection control.

Carries out emergency activities according to agency policy.

Establishes a good rapport with patients and their families, St. Johns's Home Health staff and other individuals in the community.

Follows the Mercy Home Health call-in procedure to identify whereabouts in the community.

Timely documentation and submission of all personal care reports, mileage and itineraries according to Mercy's Home Health policy.

Complies with ongoing personnel file documentation requirements.

Responsible for participation with customer service initiatives.

Maintains ongoing knowledge of and complies with Mercy and Home Care policies and procedures.

Completes required education/inservices.

Attends departmental and committee meetings as assigned.

Patients and families verbalize satisfaction with care provided. Utilizes scripting as appropriate.

Consistently adheres to universal precautions including use of personal protective equipment as appropriate.

Maintains a professional appearance, adhering to Health System dress code, consistently displaying Health System name badge.

Adheres to Health System and Home Care attendance standards.

Performs other duties as assigned.

Qualifications:

Qualifications

Education: Must satisfactorily complete the HHA competency evaluation program once hired.

Licensure: None.

Experience: Minimum of six months continuous patient care work experience within the last three years.

Certifications: Must have a current American Heart Association Heartsaver Provider Card or obtain one within probationary period.

The employee must be able to lift and/or move more than 50 pounds.

Preferred Certifications: CNA certificate preferred.
