RN Registered Nurse -- LPN -- Home Care Nurse

Job Responsibilities

As a Home Care Nurse, you will be responsible for delivering the highest quality patient care according to the specific orders of each patient's individual physician. This is a perfect opportunity for you to expand on any pediatric or adult home care experience that you may have while making a positive and much-needed difference in the lives of both children and adults as well as their families.

Specific duties for the RN / LPN role include:

Observing and assessing client condition and care needs continuously. Reporting changes in condition to the supervisor and/or physician as appropriate Developing, implementing and updating the nursing care plan Providing direct, hands-on care to assigned clients Documenting all activities, assessments, nursing actions, responses, and coordination of care in a timely manner whenever care is delivered Recognizing changes in client needs and responses requiring intervention and implementing care to prevent risk or reduce risk Accepting responsibility for personal and professional accountability by complying with PSA policies, state and federal regulations, accrediting bodies and the Nurse Practice Act Maintaining knowledge of competencies related to the nursing profession by participating in educational programs, continuing education units, Mosby skills and skill evaluations Participating in orientation of new nurses upon request. Supervising and teaching other nursing personnel Educating the client and family regarding the disease process, self-care techniques, prevention strategies, and meeting the client's nursing needs Maintaining professional boundaries at all times

Position Requirements

As a Home Care Nurse, you must be caring, patient, and adaptable with strong clinical assessment and independent problem-solving skills. It is also important that you have solid time management abilities as well as excellent communication and interpersonal skills, and an empathetic and compassionate bedside manner.

Specific qualifications for the nursing role include:

Associate's or bachelor's degree in nursing from a state accredited LPN or RN program Current and unrestricted LPN or RN license to practice in the state for which you are applying Current CPR certification Current Continuing Education if required by state Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures Proof of eligibility to work in the United States Ability to work independently and with minimal supervision Physical requirements include: Frequent lifting and repositioning of patients Repeated or prolonged standing, bending, kneeling, twisting, and occasional climbing of stairs Acute sense of sight and hearing (corrected or uncorrected) and strong sense of touch, smell, and taste Agility and strength sufficient to handle patients and equipment without assistance Ability to appropriately respond physically and psychologically to emergency situations in the home or during transport Ability to function in a wide variety of environments which may involve exposure to allergens and other health conditions Pediatric home care background, preferred
