This is a field based position with a home based (telecommuting) office.  You will work from home when not in the field.  This is a Monday – Friday, daytime hours position.  The anticipated start date for this position will be November 20, 2015.  The first 4 weeks will be core training and daily attendance is required on site at the Quad Cities corporate office:  1300 River Drive, Suite 200, Moline, IL 61265.The overall purpose of the Field Based Senior Care Advocate is to improve the enrollees ability to remain stable in the community and out of the hospital. The role is to engage the person in the treatment process and then assist him/her to access the appropriate community services so they can pursue their treatment goals in the community and avoid institutionalization.  As one of our newest Senior Care Advocates, YOU will be making field visits going into clinics and other facilities.  The Field Based Senior Care Advocate position is also designed to be a direct clinical liaison between the health plan staff and the clinical network.  The program fosters a collegial and mutually beneficial relationship between the providers and payers of medical and behavioral health services.  Responsibilities are as follows but are not limited to:

Identifies needed services and matches with available services in area
Assists enrollees with appointments
Provides subsequent member follow-up as determined by individual member needs
Works with enrollees to identify gaps in care or obstacles to care and problem solves for successful connection to needed services 
Reviews IP Census each day to determine which enrollees on the census are re-admits within the last 12 months
May go to facility to meet with treatment team to develop a successful aftercare plan
Manages the aftercare follow-up of identified high risk enrollees (manages follow-up calls/outreach and documentation)
Meets with community providers/facilities as company liaison to foster good rapport and relationships and to identify new programs
Works with CNS to identify new facilities and programs that provide services to targeted populations
Attends network development meetings 

Requirements

Required Qualifications:

Licensed Master's Degree level clinician in: Psychology, Social Work, Counseling or Marriage & Family Counseling OR a Licensed Ph.D. or Licensed Psy.D. Psychologist, with 2+ years of behavioral health experience
Nurses with a BSN and Nurse Practitioners (NP) with 3 years of RECENT psychiatric nursing experience will also be considered
Clinical Licensure must be current and unrestricted for the State of Iowa
Intermediate computer skills with the ability to navigate a Windows environment and to create, edit, save and send documents utilizing Microsoft Word
Field based experience
Experience working with a variety of populations within the community (i.e. hawk-i program, children with special needs, aged, blind, disabled, HIV/AIDS, temporary assistance for needy families) 
Access to high speed internet for home office set up
Reliable vehicle, current driver's license and proof of auto insurance Preferred Qualifications:

Dual diagnosis experience with mental health and substance abuse
Experience working with low-income populations
Experience working with the aged, blind or disabled
