Work from a list of identified members who have been discharged home
Complete post hospital assessments of the discharged members
Document the phone calls and all work within the computer database
Maintain a productivity spreadsheet detailing all of your work
Other duties as assigned

Requirements

Minimum Requirements: 


High School Diploma or GED
1+ years of experience in an office environment using the telephone and computer as the primary instruments to perform the job duties
The ability to maintain HIPAA requirements
Must be able to navigate a PC environment and utilize Microsoft Office in a professional setting
Preferred Qualifications: 


Knowledge of medical terminology
Professional experience with Medicare and Medicaid
Customer service experience
Data entry experience
