Home Health Aide

SUMMARY OF JOB DESCRIPTION:

The home health aide is a paraprofessional member of the home care team who works under the supervision of a Registered Nurse or Therapist and performs various personal care services as necessary to meet the patient's/client's needs. The home health aide is responsible for observing patients/clients, reporting these observations and documenting observations and care performed.

The home health aide will be assigned in a manner which promotes quality, continuity and safety of a patient's/client's care.

RESPONSIBILITIES AND DUTIES:

Responsibilities of the home health aide include, but are not limited to, the following:

1. Providing personal care including:

A. Baths

B. Back rubs

C. Oral hygiene

D. Shampoos

E. Changing bed linen

F. Dressing and undressing patients/clients

G. Skin care and prevention of skin breakdown

H. Toileting activities

I. Keeping patient's/client's living area clean and orderly, as appropriate

2. Preparing light snack or lunch as appropriate.

3. Assist with feeding the patient/client, if necessary.

4. Take and record oral temperature, pulse, respiration and blood pressure when ordered (within the training limitations of the aide).

5. Assist with ambulation and exercise according to the plan of care.

6. Perform range of motion and other simple procedures as an extensional therapy service as ordered (within the training limitations of the aide).

7. Assist patient/client in the self-administration of medication.

8. Assist with patient's/client's laundry, as appropriate.

9. Meet safety needs of patients/clients and using equipment safely and properly (foot stools, side rails, wheelchairs etc.).

10. Report on patient's/client's condition and significant changes to the assigned supervisory nurse/therapist.

11. Adhere to the Organization's documentation and care procedures and standards of personal and professional conduct.

12. Participate in Agency Quality Improvement activities as needed.

QUALIFICATIONS:

1. Meets the training requirements in accordance with State and Federal laws.

2. Must be at least 18 years of age.

3. Possesses and maintains good physical stamina and mental health. Presents a pre- employment physician's health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.

4. Ability to read and follow written instructions and document care given.

5. Self-direct with the ability to work with little direct supervision.

6. Has empathy for the needs of the ill, injured, frail and the impaired.

7. Possesses and maintains current CPR certification.

8. Ability to be flexible and cooperative in fulfilling role obligation.

9. Demonstrates tact, patience and good personal hygiene.

10. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

11. Minimum one (1) experience working in a home care setting.
