The Supervisor assists the Clinical Manager in developing and leading professional clinical and non-clinical staff in required activities which facilitate and support quality, cost efficient outcomes and minimizes fragmentation of healthcare delivery. The Supervisor has experience and expertise in the Case Management process and coaches the staff on the RN - patient - physician collaborative process of assessment, planning, implementation, coordination, monitoring, and evaluation to address patients holistically. The Supervisor assists the Clinical Manager to support and coordinate the operations effectiveness across the continuum of onsite and telephonic case management. The Supervisor assists the Clinical Manager to help staff to understand and demonstrate the ability to integrate the needs of consumers, customers and the business. The Supervisor In collaboration with the Clinical Manager, the Supervisor will meet with client representatives, physician practices and other members of the care system to develop relationships which facilitate communication and coordination as needed.
 
Primary Responsibilities:

Direct supervision of assigned staff
Reviews work performed by others and provides recommendations for improvement
Motivates and inspires other team members
Anticipates customer needs and proactively develops solutions to meet them
Serves as a key resource on complex and / or critical issues
Solves complex problems and develops innovative solutions

Requirements


Required Qualifications:


Current unrestricted RN license

3+ years of case management 

3+ years management experience overseeing Clinical Case Managers (RN)

 Mandatory certifications: URAC approved Case Management certification (CCM)

 Ability to travel up to 75% in the Salt Lake City area

Reliable transportation to travel to member homes or other locations within service delivery area

Proficiency with Microsoft Office 
Preferred Qualifications:


Bachelor's degree

Home care / field based case management 

Medicaid, Medicare, Managed Care experience

Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs

Experience in Hospice, Home & Community based or Long Term Care services delivery
