Overview

Responsible for the care of assigned patients for assigned shift. Performs patient initial assessments and reassessments, develops and updates patient nursing care plans. Provides direct patient care, administering medications and treatments, educating patient and care givers, and providing personal and restorative care. Performs all documentation necessary and in accordance with established policies and procedures and in compliance with all regulatory agencies. Actively participates with other healthcare disciplines in providing and improving quality care patient care services. Shares in the management of the unit by making recommendations for operational improvement, controlling costs, orientation, education and supervision of co-workers. Provides input regarding patient needs for staffing. Keeps the Charge Nurse informed of patient progress and special needs. Communicates as needed with the physicians regarding patient care. Performs all functions in an ethical and legal manner.

Responsibilities:

Responsibilities

Assessment of the Patient:

1.1 Nursing admission history and initial assessment is completed within 8 hours of admission, including assessment of a) physical; b) psycho-social; c) spiritual; d) potential for abuse, and e) pain.

1.2 Recognizes serious and life threatening situations and responds appropriately.

1.3 Assesses patient's and caregivers' ability to understand and learn self care.

1.4 Provides education for patients and family/significant other.

1.5 Interprets and utilizes diagnostic, laboratory data and information from monitoring technology to provide care.

1.6 Recognizes safety needs, physical impairments, ability to perform activities of daily living.

1.7 Ascertains presence of advance directives, and supports patient's desires for treatment measures.

1.8 Assesses needs for resources/involvement of multidisciplinary healthcare providers.

Planning of Patient Care:

2.1 Develops and documents an individualized plan of nursing care from the initial assessment which address health problems, interventions, and desired outcomes.

2.2 Utilizes the plan of care and evaluates at least q shift and updates according to on-going assessments.

2.3 Involves patient and family in plan of care as appropriate, notifies family of significant changes of condition.

2.4 Organizes work and sets priorities in care to be able to complete all tasks in a timely manner, and asks for assistance appropriately.

2.5 Communicates patient condition/needs/plan of care to on-coming shift in a timely manner.

2.6 Communicates patient condition/needs to physician in an organized manner with complete information.

Implementation of Patient Care:

3.1 Provides direct patient care according to written plan of care, and established policies and procedures.

3.2 Administers medications and treatments accurately and in timely manner.

3.3 Provides for personal care and comfort of patients, and anticipates needs.

3.4 Provides for emotional support to patient and families, especially those who are critically ill or dying.

3.5 Provides for educational needs and evaluates learning.

Evaluation of Patient Care:

4.1 Evaluates and documents patient response to care especially pain medications.

4.2 Evaluates and documents patient response and progress toward stated goals.

Leadership Skills

5.1 Delegates tasks appropriately and effectively to LPNs and CNAs.

5.2 Keeps charge nurse informed of patient needs that may impact staffing.

5.3 Exhibits behaviors that support a team effort, willing to assist and teach others.

5.4 Responds to changes in work pace calmly and with organization.

5.5 Is clinical role model for new nurses, assisting with orientation and documentation.

5.6 Exhibits ethical decision-making skills, in line with the Mission and Values, in performing all management responsibilities.

General Performance:

6.1 Attends meetings and inservices, sufficient to be current with job information.

6.2 Is responsive to staffing and scheduling needs of the unit.

6.3 Observes Universal Precautions including use of Personal Protective Equipment.

6.4 Wears name badge and maintains a clean well groomed appearance consistent with dress code .

6.5 Directions question/concerns through appropriate channel of communications.

Qualifications:

Qualifications

Education: Graduate of an accredited school of nursing.

Licensure: Must maintain current Registered Nurse licensure in the state of Arkansas or applicable Compact State License.

Experience: No prior hospital experience necessary but is desired. Three (3) months on the job training.

Certifications: Current CPR, ACLS AND PALS.

Other: Must demonstrate effective human relations and communication skills. Must possess the ability to assess data reflective of each patient's status and interpret the appropriate information needed to identify each patient's requirement relative to his/her age-specific needs to provide the care needed. Desk top computers, copiers, general office and patient care equipment.
