Must be able to work various shifts per week and be available weekdays and weekends.

Requirements

ACHIEVE is the premier provider of services to individuals with intellectual, development and other disabilities in Broome and Tioga Counties and has proudly been a part of this community for 64 years.

We are currently seeking a part-time Habilitation/Respite Provider who is responsible for providing direct care through implementation of Habilitation Plans. Habilitation/Respite Provider's specific duties and responsibilities include, but are not limited to:

* Provide training and assist consumers to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or required.
* Transport or accompany individuals on trips to community outings, appointments or recreation activities for the purpose of training in mobility, community orientation and awareness
* Foster and promote skills leading to independent living
* Assist with the development of Habilitation Plans and following these plans for training and assistance
* Provide supervision and all direct care required to maintain safety and comfort of the consumer.
* Communicate all health, safety, consumer and/or program concerns, sensitive situations and incidents to supervisor and family.

QUALIFICATIONS:
* High School Graduate with some knowledge of or experience working with the mentally retarded/developmentally disabled preferred; OR
* A combination of education and experience that in the opinion of the reviewing agency meets or exceed the above standards of the program.
* Obtain and maintain required training certifications that includes CPR, First Aid and SCIP
* Attend all required training, staff development programs and staff/agency meetings as required by the agency and/or State and Federal regulations
* Possess good interpersonal, written, oral, and communication abilities and effective time management/organizational skills
* Must physically be able to perform required certifications and lift 50 pounds
* Possess a valid driver's license acceptable to the agency's insurance carrier
