Care Management Supervisor (LCSW) needed for a permanent opportunity with Yoh's client located in Santa Cruz County, CA.

What You'll Be Doing:
- Supervise the daily activities of the Care Management staff
- Train and oversee Care Management staff in the department's daily operations
- Work with the Care Management Manager and Director to conduct process and outcome evaluation of Department's programs
- Coordinate and implement effective and efficient Care Management processes which includes Complex Care Management, Children's Care Management, Medical Social Work and care coordination
- Assist in the preparation of statistical and activity reports
- Educate new employees and conduct regular in-service training for the Care Management staff regarding updates, system or process upgrades
What You Need to Bring to the Table:
- Current California Licensed Clinical Social Worker License
- 5 years of experience in a patient care setting required
- At least 1 year of case management experience
- 2 years of experience as a full-time supervisor, or an equivalent combination of education and experience
- Knowledge of health care industry, Medi-Cal program, and managed care
- Knowledge of research, analysis and reporting methods
