The purpose of this position,  Engagement Specialist is
to provide customer support to the Disease Management Team and to the
beneficiaries of the United Health Care Military and Veterans Disease
Management Program, those who serve, have served and their families. 
 Job functions include assisting with member care coordination and
community resources integration, assist in triage and coordination
supporting UHC M&V Disease Managers by assisting with eligibility
determination, planning coordination, education and delivery of DM
services, engaging with member, families or other health care team
members as directed.

As
a potential government contractor, this position requires U.S.
citizenship and proof of favorable adjudication following submission of
Department of Defense form SF85P.
Primary Responsibilities:
Identify, engage, educate and coordinate disease  management servicesAssist with the identification of
national resources to support community based activities, coordinate and
educate referral sources within the UHG enterpriseResponsible  to assist in triage and coordination supporting UHC M&V Disease Management Managers  Program Care ManagersAssisting with determining
eligibility, planning, coordination, education  regarding Disease
Management for the appropriate membersEngaging with member, family or other health care team members as directed.Maintains accurate documentation systems/data base which assures compliance with program process and objectives and reporting.Functions independently within role with minimal need for supervisionMaintains direct and open communication within organizationsOrganizes work and develops
strategies to prioritize and adapt to constantly changing workload or
when confronted with unforeseen situationsAble to handle sensitive and confidential informationDemonstrates cultural competence and sensitivityStrong critical thinking skillsExcellent communication  skills orally and written

Requirements

Required Qualifications:
High School Diploma/GED.3+ years of Telephonic Customer Service.Ability to create, copy, edit, send and save utilizing Word, Excel & Outlook.United States Citizenship.Ability to obtain favorable adjudication following submission of Department of Defense form SF86 NAC form SF86.
Preferred Qualifications:
Associate Degree or comparable college level coursework in a Health-related field.Managed Care or experience in a Medical environment.Previous Military experience.Previous Call Center experience.

As
a requirement of UnitedHealth Group’s contract with the Department of Defense,
this position requires U.S. citizenship and proof of favorable adjudication
following submission of Department of Defense eQuip
Form SF86, (the National Agency Check Legal and Credit or “NACLC”).
Successful completion of the NACLC process is a requirement for continued
employment in this role. NACLC processing will be initiated by our TRICARE
Security Officer post-offer, and can take 3-6 months for a final decision
communication from the Department of Defense. Candidates will be allowed to
begin employment with UnitedHealth Group in this role based on an interim
clearance, and final results will be communicated as they are received. Failure
to obtain final NACLC approval will result in termination from this role.
