You'll be doing your life's best work.(sm)This position is accountable for developing and implementing the health plan’s housing navigation program, designed to connect our members with high quality, affordability housing units, resources and supports.
 
Primary Responsibilities:

Collaborating with national UHC teams to develop our local housing navigation strategy
Development of a local health plan end to end program to better connect long-term care and members with behavioral / mental health needs with affordable housing
Supporting health plan clinical and care management staff in development of assessments focused on identifying member housing needs
Initiation and development of relationships with housing-focused community organizations, landlords, developers and housing providers
Engagement with individual member cases to identify appropriate housing (such as subsidized housing, Section 8, affordable and market rate housing) members in affordable / appropriate housing, including identification and dissemination of materials, trainings and tools for care managers
Collaboration wherever possible with state agencies, regional and local governmental bodies, community organizations and other social service agencies to promote and grow housing opportunities within the state of Kansas
*Outcomes include development of a scalable and sustainable housing referral program, and increase in the number of members placed in affordable, independent housing.

Requirements

Required Qualifications:

Bachelor’s degree in Business, Finance, Health Administration, Public Policy, Social Work or related field or equivalent work experience
2+ years experience working with housing agencies, housing policy including HUD and local housing organizations and programs
Experience working with housing subsidies and vouchers, and in programs focused on helping low-income individuals and families identify the appropriate housing options
4+ year of experience in writing business and/or system requirements and business/finance analysis
2+ years of experience with documenting processes and practices, often in accepted project methodology artifacts, while effectively prioritizing multiple tasks, priorities, projects, and deadlines
Strong leadership skills and the ability to work independently and with minimal direction
Strong communication and presentation skills are required for this position, as well as process development and organization skills
Proficient with PowerPoint, Excel, SharePoint and other data tools (required)
Good at networking and building connections in the community
Experienced project / program management, including action planning, timelines and business requirement definition
Ability to work in a complex matrix environment, influencing others through communication and organizational skills
Strong understanding of Medicaid populations, providers and housing organizations,, specifically members living in nursing facilities, as well as individuals with complex needs and support including mental and behavioral health.  Sensitivity to member needs and strong person centered orientation.
Understanding of State Medicaid and/or Medicare programs, as well as Medicaid managed care
