Position Summary :

The nurse is responsible for providing comprehensive community health-nursing services through home visits to women and their families enrolled in the NFP in accord with program goals, objectives and protocols

Education/License/Degree Required:

Bachelor's Degree in Nursing, Current BLS from American Heart Association

Current Colorado nursing license

Minimum Experience/Knowledge required:

* Experience working in public health, including home visitation

* Experience in providing maternal and child health services, particularly in a community/public health setting

* Experience in working with diverse populations, communities and organizations

* Bi-lingual Spanish ability preferred

Colorado Driver's license and reliable car. Significant driving time required.

Provides nurse home visitation services to up to 25 women during their pregnancy and until the child is 2 years old services includes:

* Assessment of physical, emotional, social and environment needs of women and their families as they relate to health and life course development.

* Assisting women and their families to establish realistic goals and measurable outcomes for promoting their health and life course development

* Counseling and instruction to assist women and their families in attaining targeted goals, in areas including prenatal and postpartum care, nutrition, parenting, well-child care, family planning, and special health problems.

* Evaluation of client's and families progress toward goals; revises plan of care as appropriate.

* Development of a working relationship with clients and their families in order to promote problem-solving competence.

* Promotion of involvement of other family members in the pregnancy, birth and early childcare

* Records nursing activities in timely manner and in accord with project protocols

* Links women and families with community resources, which are relevant to their specific needs.

* Consults and collaborates with other professionals involved in providing services to women and families in the caseload.

* Assumes responsibility for ongoing learning and professional development in relation to program implementation.
