REQUIREMENTS

Must be licensed as a Registered Nurse in the state of Florida. Must have pediatric experience. A Bachelor's Degree in Nursing from an accredited college or university preferred. Must have access to a privately owned vehicle to perform duties, training and to travel to outlying facilities and be reimbursed for this travel at the state approved rate.

POSITION DESCRIPTION

Collaboration:

Effectively communicates with Clay County School District school health designees, principals, social workers, and registrars.

Collaborates with community partners through community referrals.

Conducts immunization record reviews as requested by private schools in Clay County.

Screenings:

Assists school district nurses with health screenings.

Conducts screening training with volunteers prior to school's screening date.

Ensures that school health designee completes rescreens and screening of absentees, in a timely manner.

Inputs screening data into computer program to generate evaluation letters of screened students.

Sends initial referral letters to families of screening failures.

Follows up with school nurse regarding outcome of referrals.

Makes third contact with parents that have not responded to previous referral letters.

Participates in screenings as requested by private schools in Clay County.

Trainings:

Train registrars and school health designees on current immunization and school entry health exam requirements.

Conducts 4 hour orientation class for new school health designees.

Performs child specific training with Unlicensed Assistive Personnel (UAP) in schools without a Registered Nurse.

Provides annual updates on school health policies and procedures.

Program Oversight:

Visits assigned schools to ensure adequate program oversight.

Performs biannual reviews of each school health room using the current QI tool.

Reports QI findings to appropriate school personnel.

Maintains Policy and Procedure book.

Assists with analyzing and inputting of data/statistics for annual school health report.

Meetings:

Active member of the School Health Advisory Committee (SHAC) and community committees as assigned.

Updates school health team regarding community committees.

Attends 504 and IEP meetings as requested by the school district, in schools without a Registered Nurse.

Participates in school health services annual meetings.

Other duties and responsibilities:

Provides back to school outreach events with community partners.

Provides completed DH 680 forms for schools after immunization data entry into Florida Shots.

Resource to school health staff regarding health education by providing health education material.

Creates care plans for students at schools that do not have a Registered Nurse.

Completes records, reports and coding for continuity of school health services.

Codes clinic activity reports from previous month into the Health Management System (HMS) by the 15th of the current month.

Communicates with parents and other professionals by telephone, home visits, or letter for follow-up of health problems.

Assists DOH Clay Epidemiology Program with outbreak investigations upon request.

Attends in-service workshops, seminars, and staff conferences to update professional knowledge and skills.

Performs other related duties as required/assigned and within the realm of licensure, protocols and the classification specifications of a Senior Community Health Nurse.

Serves as preceptor for new employees.

Responsible for securing the designated information sets for the purpose of protecting confidentiality, data integrity and the appropriate access for all information, both confidential and public record, stored in hard copy or electronic format.

Participates in providing on-call coverage as assigned.

DESCRIPTION

Delivers infants and performs postpartum examinations and treatment.

Refers students or patients to community agencies furnishing assistance and collaborates with agencies.

Conducts specified laboratory tests.

Maintains stock of supplies.

Directs and coordinates health care programs.

Develops, negotiates and implements public/private partnerships.

Performs grant writing and/or contract management activities.

Conducts inspections or programmatic evaluations and writes reports of findings, recommendations and corrective actions.
