About this job

Responsibilities* To provide nursing care, assessment, evaluation, training and monitoring to an assigned patient population (inclusive of pediatric and adult) requiring home infusion and durable medical equipment prior to hospital discharge and in the outpatient clinic setting.

To provide nursing advice and consultation to nursing and other health team staff along the continuum of care.

Triage problems as they arise with home patients.

To train and serve as a preceptor of nursing and others assigned to Home Care Services.

To participate in health team activities.

Nursing Specific Info Salary & Nursing Framework Level:

This UMPNC RN posting is posted as Nursing Framework LEVEL C.

Actual Nursing Framework LEVEL and salary will be determined at time of hire.

Nursing Framework levels range from Level A to Level E.

Required qualifications must be met by the candidate in order to be interviewed and considered for the position.

Required Qualifications* Current license to practice as a Registered Nurse in the State of Michigan.

Two (2) or more years adult and/or pediatric medical/surgical nursing experience within the last five (5) years.

Two (2) years of recent clinical experience that demonstrates effective coordination of clinical care for a broad range of patients.

Ability to demonstrate excellent problem solving, critical thinking and triage skills and to work independently in a multidisciplinary, fast paced environment as evidenced by interview, references and performance evaluation.

Direct care with all types of vascular access devices (i.e.: PICCs, midlines, implanted, tunneled), wound care and oxygen patients. In-depth recent experience in patient education (i.e.: self care, disease state management).

Excellent communication and customer service skills.

Demonstrated knowledge of the continuous quality improvement process.

Desired Qualifications* Home Infusion, DME and \or home care experience as a RN within the last two (2) years.

BSN preferred

Experience teaching and developing patient education material/policies and procedures for home infusion and equipment techniques.

Two (2) or more years adult and pediatric medical/surgical nursing experience.

Successful completion of chemotherapy course.

Certification in Infusion Therapy or Nutrition Support.

Computer skills, i.e. Microsoft Word

Experience with McKesson

Peripheral IV insertion experience

Work Schedule Hours per week: 40hrs/week

Shift/ Hours/Days: Hours 11 AM to 7:30 PM, but may vary. Occasional on-call

**Note: Role will require cross training to both the Hospital staff role (in-hospital setting) and in-home nursing (patient visits).
