Social Worker BSW - Full-time / Part-time

Providence is calling a Social Worker BSW to Providence Seward Medical and Care Center in Seward, AK.

Social Worker works with elders, families, guardians, members of the community, physicians, psychiatrists, health care professionals, community members and community resources, to provide medically-related social services to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.

In this position you will:

Manage a caseload and maintain an organized, current case file system that reflects the psychosocial needs of the elder, and the efforts made to meet those needs.

Complete social histories, social assessments, and care plans for each elder at admission. Ensure that the elders comprehensive care plan and elder daily care plan include interventions that address social service needs.

Arrange for and coordinate the discharge or transfer of appropriate elders to their homes or other community placements, and assure continuity of care.

Maintain and enhance professional competence and personal growth through on-going education, in-service training, workshops and seminars.

Adhere to the professional guidelines and principles of NASW standards.

Maintain confidentiality of all elder medical and personal information or elder and facility financial information obtained while performing the duties of the job and from anyone who does not have authorized access to such information.

Required qualifications for this position include:

Bachelor's Degree in Social Work

Alaska Licensed Baccalaureate Social Worker (LBSW)

Ability to assess psychosocial behavior and medically related resident needs, and develop a meaningful plan of care, appropriate interventions, and effective discharge plans

Ability to effectively communicate with residents, families, other professionals, and the community, in a positive, professional manner

Ability to articulately express in writing, social work interventions, resident assessments, plans of care, and place these in the record in a timely manner as required by facility policy

Preferred qualifications for this position include:

One (1) year of recent experience in a medical facility or long term care setting
