Job Summary

This facility-based position initiates and coordinates incoming transfers requiring a higher level of care and/or specialized treatment modalities from outreach or network facilities. Secures required specialists, assessment and triage for appropriate level of care, financial assessment and referral.

Essential Functions Coordinates patient placement to outreach and network facilities. Alerts staff of incoming patients, scheduling bed/services, performs financial and clinical screening of referred patients, and coordinates transportation. Serves as liaison to other facility departments, shifts, and presiding physicians regarding process improvement for patient flow, patient transfers, and efficacy of communication process.

Acts as a liaison to outreach and network patients and their families when applicable by coordinating subsequent travel, lodging and education of patients procedures. Serves as the liaison for the continuum of care including preadmission, hospitalization and community follow-up based on facility needs.

Maintains process for assessing current referral patterns for selected target areas by identifying physician referral patterns, health related community events, established physician practices, and major gatekeepers in outreach and network communities. Continually assesses the changing outreach and network physician landscape.

Serves as a resource and provides facility leadership assistance to achieve optimal clinical, operational, financial, and satisfaction outcomes. Acts as a consultant within the organization and in the community.

Serves as a resource to other staff members by promoting a collegiality between staff, physicians and the company leadership teams.

Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies. Maintains current knowledge of federal, state, local, facility laws, regulations, policies, and procedures including EMTALA, COBRA, and STARK.

May supervise staff depending on work assignment.

Minimum Qualifications

Requires knowledge typically obtained through an associates degree in nursing or an equivalent level of education and experience.

Requires current RN license in state of practice.

Requires knowledge typically achieved with two years of experience in the area of clinical operations, case management and patient throughput. Must have a working knowledge of hospital operations, medical/nursing staff procedures, hospital and community resources. Must demonstrate critical thinking, conflict resolution, time management, and problem-solving skills. Requires excellent communication skills and an ability to interact well all staffing and leadership levels within the facility. Excellent organizational and human relations are required to maintain good rapport and effective working relationships with internal and external customers.

Preferred Qualifications

Bachelors degree in nursing.

Additional related education and/or experience preferred.
