Sutter Care at Home is the home health care and hospice affiliate of Sutter Health, one of the nation's largest not-for-profit health care systems.

Under the supervision of the Director of Patient Care Services, the Team Nurse/Therapist is responsible for the delivery of appropriate, adequate, effective, efficient direct patient care services assigned by the Primary Care Clinician and approved by the Director of Patent Care Services. Responsibilities include implementation of the physician plan of care, obtaining physician orders, supervision of home health aides, and coordination of care within an interdisciplinary team and participating in effective utilization management and control of patient supplies. Consistently provides care based on the age-related needs of the unique population with consideration of their development/psychosocial needs, environmental safety, educational needs, communication style, and level of understanding. The Team Nurse/Therapist is accountable to the Primary Care Clinician for ensuring that the plan of care is implemented following professional standards of care and agency specific protocols. Education RN, PT, OT or SLP graduate from an accredited curriculum. Bachelor Degree in nursing or health related field preferred. Master's degree required for SLP. Current California RN, PT, OT, SLP license, valid California Driver License with proof of current auto insurance, current BCLS CPR certification. Experience RN and SLP's One year of professional experience strongly preferred. One year of experience in a home health or hospice position preferred. PT and OT's must have minimum of one year of appropriate experience strongly preferred. One year of experience in a home health position preferred. Knowledge Exhibits excellent oral and written communication skills with legible handwriting for documentation of patient records in a clear and concise manner. Basic understanding of home care services, clinical standards of care and practice, Title XXII, Medicare Conditions of Participation, JCAHO home health accreditation requirements, QI outcomes/benchmarking, infection control. Good understanding of reimbursement patterns for Medicare and other payors. Good working understanding of budget and other financial reports.
