Primary Counselor I - GENERAL SUMMARY :
Provides the services to adults including: conducting face-to-face assessments, establishing diagnoses, making treatment suggestions; ordering remedial services (when indicated); and assisting with referrals to remedial providers (if requested); Provides substance abuse, co-occurring trauma, and personal growth-related therapy services to assigned clients and their families. Applies advanced psychosocial theory and methods to individuals as well as families and groups with appropriate regard for the complexity of given problems. Performs tasks and makes decisions that require independent judgment, are complex in nature, involve short and long range planning and require the use of high level problem solving techniques. Applies concepts of client- centered care to improve client outcomes. Coordinates internal/external services to provide as complete a continuum of care as possible.
ESSENTIAL FUNCTIONS :

Obtains bio-psychosocial history information and identifies client needs by interviewing the client, reviewing available records, conferring with other professionals and appropriate House of Mercy staff, and contacting other information sources as necessary and appropriate. Analysis and evaluation of client situation consistently reflects advanced competency in psychosocial assessment and diagnosis. Assesses consistently reflects advance competency in psychosocial assessment and diagnosis. Assesses problems appropriately in terms of scope, consequences and needed interventions.
* Performs a comprehensive assessment (psychosocial history) to analyze and synthesize the client's status and addresses the client's strengths, problems and areas of clinical concern. On an ongoing basis, assesses client needs/strengths, behaviors, and progress toward desired outcomes. Specifically monitors client progress in areas of personal growth, education, prenatal care, parenting, employment, living skills, etc. Determines need for outside professional evaluations (e.g. mental health), and independently refers to community practitioners/agencies as needed.
* Develops treatment plans based upon assessment and in accordance with program requirements, state regulations, etc. Communicates pertinent treatment plan information to clinical staff and others as needed. Implements plan and monitors patient's progress.
* Participates in case review meetings to review and discuss client progress, assure consistent implementation of treatment plan goals/objectives, and promote team involvement in problem solving/resolution.
* Coordinates services with community agencies, schools, courts, service providers of significant others, and mental health/other health care professional to provide an extended, high quality continuum of care for House of Mercy adult residents.
* Consults with House of Mercy staff and client to determine client's post-discharge needs and plans. Coordinates services with community agencies as needed to provide an extended, high quality continuum of care, a groad spectrum of discharge planning services and collaborates with treatment team in implementing appropriate discharge plans and aftercare programming. Provides support, encouragement as needed throughout the process.
* Maintains an awareness of applicable laws, regulations and licensing requirements as they relate to resident documentation. Assumes accountability for providing appropriate documentation of all therapy/counseling sessions, significant incident occurrences, phone conversations, etc. consistent with applicable regulatory requirements.
* Prepares and distributes required reports and other documentation of services according to House of Mercy policies and procedures, applicable laws, regulations and licensing requirements.
* Responsible for the maintenance of client files. Assures all needed releases of information and other paperwork are present in the file and up-to-date. Promptly requests new or additional information and paperwork as needed.
* Assures that Resident Counselors, Adult Programs Counseling Services and Internship/Staff education Supervisor, Director and other House of Mercy staff (as appropriate) have the information they need to provide quality service to the client; assists in the training of
Resident Counselors.
MARGINAL FUNCTIONS :
* Provides statistical and research information as requested and/or required by laws, regulations or licensing requirements.
* Participates in House of Mercy's Performance Quality Improvement Process.
* Participates in public relations and media efforts to promote House of Mercy; attends community and informational meetings to keep House of Mercy current and to network with other agencies; attends training on relevant topics to stay current in meeting requirements of licensing and other regulations, and to obtain information to better serve clientele.
* Participates in the education and training of residents, students and providers in coordination with supervisor.
* May drive locally.

* Valid Driver's/Chauffeurs License required, must meet Mercy's Motor Vehicle Safety Standards, must be at least 18 years of age and be eligible to drive per Iowa state law.
* Proof of completion of Mandatory Reporter abuse training specific to population served within thirty (30) days of hire.
* Licensed to provide mental health therapy in the State of Iowa (LMHC, LISW, LMFT, etc.) and meets the requirements necessary for an LPHA designation OR
* CADC certification (or obtained within nine (9) months of hire), required.
* Ability to understand medical terminology and communicate effectively with healthcare professionals.
