Position Summary: Responsible to ensure that each resident in Memory Care has a successful pleasant day including appropriate supportive services for activities of daily living as well as opportunities to engage in programs and meaningful interaction with others. With the support of the Senior Lifestyle Legacy Court Caregiver, each resident will be well nourished, well groomed and actively engaged in a meaningful day. Essential Functions: * Provides kind, caring, and consistent companionship and support. * Follows the schedule of resident's care and activity needs set out by supervisor. * Learn the resident's life history, preferences, daily routine and activity interests in support of person-centered care giving. * Actively engage residents in meaningful interactions and experiences o Provides assistance, encouragement, and restorative interventions, as needed, for Legacy Court residents to be as independent as possible in their activities of daily living including and not limited to: bathing, dressing, grooming, toileting, assist in dining, and mobility. o Provides and documents response to, group and individual, therapeutic activities. * Becomes an active participant in the Walk with Me program * Communicates and documents any changes in resident's condition: o Cognitive: memory, speech, problem solving, response to instructions/ requests o Behavior: tearfulness, withdrawal, agitation, wandering, combativeness o Physical Health: vision, hearing, mobility, acute illness o Participation in Activities: interests, preferences, connections * Takes and records blood pressure, pulse, and respiratory function and weight, as appropriate. * Serves as host and hostess during mealtime; documents food and fluid intake. * Maintains safe environment in resident rooms, common areas, non-resident areas, and storage of supplies and equipment. * Provides emotional and social support to residents. * Contribute to the development and implementation of the resident activity calendar with direction from the Legacy Court Director or designee. * Assists in maintaining Life Station engagement areas throughout the neighborhood. * Documents daily log of care assistance. * Informs supervisor of any resident issues or concerns. * Respects and encourages the independence and dignity of the residents. * Respects residents' confidentiality. * Is familiar with emergency equipment and procedures * Reports issues and concerns expressed by family members to the Director Attends all required training, in-service, and staff meetings. * Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. * Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers. * Adheres to all policies and procedures of Senior Lifestyle Corporation. * Performs other duties as assigned. Qualifications/Skills/Educational Requirements: * A High School Diploma or General Education Degree; or one to three months related experience and/or training; or equivalent combination of education and experience * May require a CNA or CHHA licensure
