The goal of the Behavioral Health Field Care Advocate  role is to help individuals live their lives to the fullest by promoting recovery and resiliency via coordination and collaboration with multiple internal and external partners including consumers and their families/caregivers, medical, behavioral health and clinical network teams.Field Care Advocates work with complex and high-risk needs with a goal of engaging the consumer in the treatment process, decreasing their reliance on higher levels of care, helping them to access appropriate community services, and assisting them in improving community tenure.
 
This is a field based position with a home based (telecommuting) office. You will work from home when not in the field. This is a Monday – Friday, daytime hours position. Candidates will travel to facilities and homes within the South/Central Louisiana County area.
 
Candidates will travel to facilities within the region/county to meet with high risk Medicaid patients to do the assessment. They will then focus on identifying gaps in the care plan and work with the families/guardians to make sure that these patients are receiving the services they need.Primary Responsibilities:

Performing care management activities to ensure that patients move through the continuum of care efficiently and safely
Assessing and interpreting customer needs and requirements
Reviews cases and analyzes clinical information in conjunction with Medical Directors to determine the appropriateness of hospitalization
Performing Clinician to Physician interaction to acquire additional clinical information or discuss alternatives to current treatment plans
Escalates cases to the Medical Director for case discussion or peer-to-peer intervention as appropriate
Performs anticipatory discharge planning in accordance with the patient's benefits and available alternative resources
Collaborates with Medical peers on consideration of discharge planning needs
Refers patients to disease management or case management programs
Assists with the development of treatment plans
Documents activities according to established standards
Identifies solutions to non-standard requests and problems

Requirements

Required Qualifications:

Licensed Master's Degree level clinician in: Psychology, Social Work, Counseling or Marriage & Family Counseling; or Licensed Ph.D. or Licensed PsyD; OR an RN with at least 2 years of experience in behavioral health
Licensure must be current and unrestricted
Minimum 2 years of behavioral health clinical experience in an inpatient/acute setting or outpatient setting
Intermediate computer skills (Microsoft Word, Outlook and Internet) with the ability to navigate a Windows environment and to create, edit, save and send documents utilizing Microsoft Word
Access to high speed internet services and dedicated space for home office set up
Preferred Qualifications:

Case Management experience
Knowledge of public sector in Louisiana
A background that involves utilization review for an insurance company or in a managed care environment
