The Field Based-RN functions as part of the primary care team, and reports to the Clinical Services Manager.  The Field Based-RN supports the team by performing activities as outlined below, and as assigned by the Clinical Services Manager of the team.  The Field Based-RN performs activities that fall within the RN scope of practice.  This role works in close collaboration with all of the team members, and may support multiple providers. The Field Based-RN works under the direction of the Clinical Services Manager, and activities are delegated to the RN by the provider or the team CSM. The Field Based-RN understands and supports the ISNP model of care, and works in collaboration with the team to meet the goals of the program.
 
 
Primary Responsibilities:

Provide in-services to partnering long-term care and nursing home staff for development 
Assist Nurse Practitioners with health assessments and case management activities
Obtain vaccination reports from nursing homes and enter information into patient electronic medical record
Travel to nursing home facilities daily throughout Macon and Milledgeville, (Bibb/Baldwin) counties
Transcribe on-call logs into electronic medical record  
 

Requirements

Required Qualifications:

Registered Nurse (RN) with an active and unrestricted license in state of Georgia
2+ years clinical experience as a RN in a hospital, acute care, home health, hospice, or nursing home.

Access to reliable transportation that will enable you to travel to client or patient sites within a designated area
Strong computer skills (Microsoft Office:  Word/Excel) and competency with electronic medical records
Preferred Qualifications:

2 years of long-term care or nursing home experience
Experienced in ICU, CCU or ER settings
Skills:

Experience with using an Electronic Medical Record
Excellent communication, customer service skills and critical thinking skills
Must be self-motivated/self-directed
Ability to work independently
Solid patient assessment ability
