Join us and help people live healthier lives while doing your life's best work.(sm)   The Care Coordination Manager position is responsible for the supervision and oversight of a team of licensed staff in providing coordination of care for consumers moving from higher to lower levels.  The Care Coordination function addresses consumer complex and high-risk  behavioral and psychosocial needs, promotes consumer engagement in the treatment process, links consumers to community services, and assists consumers in improving and maintaining community tenure.  The manager ensures that the team is functioning at optional effectiveness and efficiency by continuously monitoring team performance and taking corrective action when necessary.  The manager should be a mentor and role module who encourages the team to strive toward excellence and professionalism.                Primary Responsibilities    Managing a team of behavioral health licensed clinical staff  (Field Care Advocates), to deliver sound clinical interventions and quality documentations    Serve as an behavioral health care coordination representative and subject matter expert for UnitedHealth Care Community partners, psychiatrists and other clinical and program staff to ensure that all care coordination programs are managed according to state requirements      Work closely with the clinical operations teams to identify and address the clinical needs of individuals accessing behavioral health servicesPreparing reports and analyzing data in order to coach team to meet metrics successfullyIdentifying trends and interventions as needed in a positive, change oriented environment in order to promote consumer recovery via a recovery, health and wellness-oriented approach   Local and multiple state travel 10-15%, or, as neededManaging projects and interventions from inception to completion  Ensuring engagement and productivity of staff through management of ongoing auditing, training, performance management, coaching and mentoring     

Requirements

Required Qualifications:     Master's degree in Psychology, Social Work, Counseling or Marriage or Family Counseling, or an RN (BSN or MSN) with 2+ more years of recent experience in psychiatric nursingPossesses an active, independent, and unrestricted licensure and certification from the state or board which issued the credentials5+ years of behavioral health experience post licensure5+ years of management and/or leadership experienceExperience working with Medicaid clinical programs Proficient in Microsoft Applications to include Word, Excel and Outlook  Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area Ability to travel to designated states (10-15%), or, as needed     Dedicated space for home office set up and access to high speed internet services.             Preferred Qualifications:     Experience in managing multiple clinical programs, across multiple states     Experience managing field based/telecommuting employees     Experience working in care coordination or case managementExperience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients Dual diagnosis experience; mental health and substance abuseMedical/Behavioral setting experience (i.e. hospital, managed care organization, or joint medical/behavioral outpatient practice)Experience working with low-income populations, the aged, blind and disabledManaged care and utilization management experience              Careers with Optum.
