The Case Management Team Lead is responsible for understanding and performing the roles and responsibilities of the clinical staff and their functions. Additionally, the Clinical Team Lead collaborates with internal and external customers and assists the Clinical Manager with managerial activities as needed.  You will be supervising a team of approximately 18 telephonic Case Managers / Disease Managers onsite in our Tonawanda office location at 500 Colvin Woods Parkway, Tonawanda, NY.  The Case Management / Disease Management team is responsible for inbound and outbound calls, working with our Commercial clients.  This is a Monday - Friday position during normal business hours.  Three nights per month are required to work until 8pm.  This position is onsite in our Buffalo/Tonawanda location.

Primary Responsibilities:

Provide coaching to clinical staff
Maintains an understanding of the roles and responsibilities of the clinical staff and their functions, and executes those roles and responsibilities as needed
Assists the Clinical Manager with managerial responsibilities as called upon
Manages escalated issues from internal and external sources
Acts as a resource to clinical staff for consumer issues, program definitions, and clarification
Performs as liaison for customer and vendor issues
Special projects, initiatives, and other job duties as assigned
Monitors client performance results
Maintains multistate licensure as telephonic outreach to members may be required as dictated by business need
This position will require active and unrestricted Nursing licensure in multiple US States.  Selected candidate must be willing and able to obtain and maintain multiple state licensure. (Application fees and filing costs paid for by UHG)   
 

Required Qualifications:


A current, unrestricted RN license in the State of NY

Multiple state licensure (in addition to Compact License if applicable) or ability to obtain multiple state nursing licenses

Minimum of 3 years of clinical experience

Case management / Disease management experience

Obtain CCM within 1 year of assuming the position if you do not already have one
Preferred Qualifications:

Bachelor's degree or greater
Prior Leadership experience to include Team Lead, Supervisor or Manager
CCM
