Multiple positions available!

Licensed Nurse (RN or LPN) in State of New Jersey Clinical Experience (hospital, provider office, nursing/rehab facility) Expertise with data management (charting, Excel)

Effectively engage providers and care management professionals and positively affect strategic outcomes related to provider practice pattern and member behaviors.

This position requires face to face interactions as well as telephone communications.

Performs medical abstraction in support of project initiatives Maintains productivity level as determined by management Accurately enters data from medical records into a database Ensures HIPPA and information security are maintained at all times Excellent communication skills, written and verbal, with various levels of healthcare delivery system, PCPs, Specialists, Nurse Case Managers



Work Environment:

Candidates will be working from home. The central location will be in Newark, NJ. Candidates will be visiting doctors offices to collect charts and discharge information to work to identify gaps within discharge and re-admittance.

Qualifications:

At least 1-2 years of clinical experience

Graduate of accredited US nursing program (LPN or RN)

Unrestricted and active New Jersey nursing license
