The transitional care nurse practitioner (NP) conducts in-home, and in clinic/acute facility (ex. SNF) health assessments with a focus on providing care to the most fragile, medically complex elders and others with chronic debilitating medical conditions. The goal of the NP is to provide comprehensive care management to the patient and their family/caregiver(s) in order to improve quality and efficiency of care, maximize quality of life and reduce redundant or unnecessary services. The NP will bring primary care and related health and psycho-social support services into the clinic/acute care facility and homes of frail, low mobility patients in an appropriate, coordinated manner. The NP also provides education and support to caregivers and family members in the challenging and difficult task of caring for their loved ones in the home.

Primary duties may include, but are not limited to:

Caring, compassionate attitude, critical thinking skills, excellent verbal and written communication skill, ability to adapt to a changing environment, self-motivated and flexible, collaborative relationship with multiple members of the healthcare team. Ability to manage patient care in the SNF setting, to include interdisciplinary care, appropriate referrals, medical management and assessment Judiciously and appropriately refer the patient to needed services during transitions of care Assess patients in the acute facility and develop treatment and plans tailored to the patient's condition Competent in performing safety evaluation w/in patient living environment/clinic/acute setting and providing recommendations for safety and community resources. Competent in discussing end of life/palliative care planning with patients coping with advanced stage and terminal illnesses. Completion and signature of POLST on each patient encountered. Evaluate nutritional needs and deficits and ensures follow-up continuity of care Administer medications, perform minor procedures with in scope of practice Competent in administering and interpreting mental health examinations such as: Mini-Mental Status Examination for dementia and PHQ-9 for depression. Competent in Symptom Assessment and management (ex ESAS) Educate members on topics such as disease process, medication, and compliance Complete any duties as assigned by the Regional Clinic Manger (RCM) Participates in On-Call program which may include after-hours, weekend and holiday calls and visitations.

Qualifications

Masters Degree in Nursing Required Current unrestricted RN license and NP license in applicable state required. Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background. National Board Certification within 1 year and if applicable in your governing state. 2-5 years of experience working in acute care settings such as: ICU, CCU, telemetry, medsurgical is highly preferred. Prescribing authority/DEA licensure in good standing as required in the state in which you are applying preferred. Spanish speaking highly preferred Ability to travel to multiple patient home sites (5-6 patient homes per day) to perform home assessments. Average of 40-80 miles per day. While performing the duties of this job, the employee is regularly required to stand, walk, sit, bend, kneel and occasionally lift patients.
